1. Why do I need to fill out a Form 6?

It is a way for you to tell us the details of what happened to cause the injury or illness.  It also provides us with information we need to make decisions about and process your claim. This form is different from the one you may have filled out at work for your employer.

2. Do I need to send in the Form 6 if the employer already sent in the Form 7?

Yes, please submit the Form 6 information -- either online or mail it in (not both). Your obligation to report is separate from your employer's. Also, your employer is legally obligated to provide you with a copy of the Form 7. If you did not get it, call your employer and request your copy. If you have any problems contact your claims adjudicator.

3. What Internet browsers can I use to access online applications?

The WSIB eService applications were designed for and work best with:

  • Internet Explorer 6.0 or higher
  • Adobe Acrobat 6.0
  • 1024 x 728 screen resolution

4. Is the eService secure?

 

The WSIB takes reasonable steps to protect information sent electronically. This includes use of Secure Socket Layer (SSL) encryption, to protect against loss, theft, and unauthorized access, use, modification or disclosure.

6. How do I "sign" the electronic Form 6?

 

Because you cannot actually sign the electronic form we need you to read the declaration and check the box to confirm that the information you have submitted is true. Checking the box indicates that you have read and agree to the declaration statement, and when you provide your name, it replaces the signature required on the paper form.

By doing this, you are also giving permission to the health professional treating you to provide you, your employer and the WSIB with information about your functional abilities, which can be used to help you return to work safely. The WSIB may contact you to confirm or clarify information you submitted, as well as to request any missing or additional information.

7. How will I know if the WSIB has received the Form 6?

The system will provide you with a:

  • four digit confirmation number and the time and date we received the report
  • final version of the completed submission that you can view/save or print in PDF format, and
  • the confirmation page tells you that the WSIB has received your submission.

We recommend that you:

  • print the confirmation page showing the time, date and confirmation number for your records AND
  • save or print a copy of the completed PDF form for your records since it represents everything you have submitted and has your confirmation number and the date and time we received your submission.

DO NOT save the PDF version of the form on a public computer. It will allow others with access to the computer to view your personal information.

8. Why am I not able to sign in?

Check the following:

  • Do you have the claim number? Is it correct?
  • Did you use the correct date of birth - enter it the same as on the letter you received?
  • Have you correctly typed in your date of birth (DOB) - Day/Month/Year (dd/mm/yyyy) as opposed to Month/Day/Year (mm/dd/yyyy)?
  • Is the incident date more than one year from today's date?
  • If the system recognizes your age as under 16 you have to report using the paper form as you will need your guardian's signature as well as your own.

9. Will I get paid faster? Will my claim be expedited faster? How long do I have to wait?

Electronic submission is an easier and more efficient way for you to submit your Form 6. This will allow us to have the information available to your Adjudicator within 24 hours.
